عنوان فارسی اضطراب مرگ پرستاران بخش مراقبت ویژه کووید-۱۹ در بیمارستان های وابسته به دانشگاه علوم پزشکی تهران
چکیده فارسی مقاله زمینه و هدف: با توجه به تاثیرات روانشناختی عمده همه گیری کووید-19، علاوه بر جمعیت عادی، مراقبان سلامت از جمله پرستاران نیز با چالش های فراوان روانشناختی از جمله اضطراب مرگ مواجهه شده اند. اضطراب مرگ می تواند منجر به بروز اختلالات روانی، تضعیف سیستم ایمنی و کاهش کیفیت مراقبت های ارایه شده و کاهش رضایت شغلی پرستاران شود. با توجه به ماهیت تنش زای بخش مراقبت های ویژه و ضرورت توجه به ابعاد روانشناختی پرستاران و تاثیرات مخرب همه گیری کووید-19، این مطالعه با هدف تعیین میزان اضطراب مرگ پرستاران بخش مراقبت ویژه کووید-19 انجام شد.
روش­ تحقیق: این پژوهش یک مطالعه توصیفی مقطعی بود که در مراکز آموزشی درمانی وابسته به دانشگاه علوم پزشکی تهران در سال 1399 انجام گرفت. جامعه مطالعه‌ی حاضر را 179 نفر از پرستاران شاغل در بخش مراقبت ویژه کووید-19 مراکز آموزشی درمانی وابسته به دانشگاه علوم پزشکی تهران شامل مجتمع امام خمینی ره و بیمارستان ولیعصر تشکیل داده‌اند. در این مطالعه از روش نمونه‌گیری مستمر استفاده گردید. جمع آوری داده ها با پرسشنامه اطلاعات جمعیت شناختی و پرسش نامه 15 گویه ای اضطراب مرگ تمپلر انجام شد. داده ها با استفاده از نرم‌افزار SPSS نسخه24 و روش های آمار توصیفی و استنباطی تحلیل شد. میزان معنی داری  (0/5>P) در نظر گرفته شد
یافته­ ها: یافته های حاصل از این مطالعه نشان داد که میانگین اضطراب مرگ پرستاران 2/007 ± 8/74 بود که با توجه به دامنه نمره 0 تا 15، میانگین نمره کسب‌شده بالاتر از میانه‌ی ابزار است. همچنین نتایج آنالیز واریانس یک‌طرفه نشان داد اضطراب مرگ تنها با سن پرستاران (0/001>P) ارتباط معنی‌دار آماری داشت.
نتیجه‌گیری: براساس نتایج حاصل از مطالعه، پرستاران شاغل در بخش های ویژه در زمان همه گیری کووید-19 اضطراب مرگ زیادی را تجربه می کنند. بنابراین لازم است که سیاست گزاران، مدیران آموزشی، درمانی و بیمارستانی با توجه به یافته های حاصل و همچنین نظر به عوامل موثر بر اضطراب مرگ، برنامه های مدون آموزشی و مداخله ای از جمله برنامه های ضمن خدمت، انجام مشاوره جهت کاهش اضطراب و افسردگی را در دستور کار قرار دهند تا از این طریق بتواند راه را برای پرستاران جهت غلبه بر شرایط موجود فراهم آورند.

Background: Considering the major psychological effects of the Covid-19 pandemic, in addition to the general population, health care workers, including nurses, have also faced many psychological challenges. This epidemic has exposed more nurses to occupational stress and anxiety than ever before, which has affected various aspects of their work and life, including the quality of care and their quality of life. One of the important states that the Covid-19 pandemic can intensify is death anxiety. The fear and anxiety caused by this disorder can impose a destructive and major psychological burden on a person, including nurses, and lead to the occurrence of mental disorders, weaken the immune system, and affect the quality of care provided, as well as the job satisfaction and psychological state of nurses. Considering the importance of paying attention to the psychological dimensions of nurses' lives, especially considering the limitations and destructive effects of the covid-19 pandemic, especially nurses working in intensive care wards, considering the stressful nature of these wards, and the need to know the different dimensions of death anxiety and factors Affecting it, as well as the lack of conducting sufficient studies in the existing conditions and contradictory results, this study was conducted with the aim of determining the death anxiety of nurses working in the intensive care units of the covid-19 medical training centers affiliated to Tehran University of Medical Sciences in 2021.
Method: This research was a descriptive cross-sectional study in which the death anxiety of nurses working in the intensive care units of the covid-19 medical training centers affiliated to Tehran University of Medical Sciences 2021. The population of the current study was composed of 179 nurses working in the intensive care units of the covid-19 in Tehran and Qom, hospitals... and... Continuous sampling method was used in this study. In the city of Tehran, there were nurses from the hospital..., and in the city of Qom, there were nurses from the hospital... After sending the questionnaire link, 179 nurses answered the questions completely. The approximate time to complete the questionnaire was 5 minutes. Data collection lasted from December 5 to March 16, 2019. In order to collect data, a demographic information form was used to measure information such as age, sex, marital status, work history, and level of education, and Templer questionnaire was used to check the level of death anxiety. Death anxiety questionnaire by Templer consists of fifteen items in a yes-no form. The range of scores of the questionnaire is 0-15, and a higher score means more death anxiety. In this study, data analysis was done using SPSS version 24 software. In the descriptive statistics section, frequency distribution tables were used for qualitative variables of the research and numerical indicators of minimum, maximum, average and standard deviation for quantitative variables such as death anxiety. In the inferential statistics section, independent t-tests and one-way analysis of variance were used to correlate death anxiety with demographic information.
Results: The results of this study showed that the average death anxiety of nurses was 8.74 ± 2.007, which according to the range of scores from 0 to 15, the average score obtained is higher than the mean of the scale. Also, the results of one-way analysis of variance showed that death anxiety had a statistically significant relationship only with the age of nurses (P=0.000). Tukey's two-way comparison showed that the average score obtained in death anxiety of nurses with an age range of 30-39 was lower than that of nurses 40 years and older and nurses less than 30 years old. It is worth mentioning that other demographic information of nurses had no statistically significant relationship with death anxiety (P<0.05).
Conclusion: This study was conducted with the aim of determining the death anxiety of nurses working in the intensive care units of covid-19 medical training centers affiliated to Tehran University of Medical Sciences in 2021. The results showed that nurses working in special wards experience high levels of death anxiety during the Covid-19 pandemic. The obtained results double the importance of paying attention to the dimensions of psychological health of nurses, especially nurses working in intensive care units due to the exceptional working conditions in these units. Especially in the conditions of the covid-19 epidemic, when these employees are more susceptible than ever to suffering from psychological disorders, and as a result, their personal and professional lives are affected, especially the quality of care provided. Therefore, it is necessary that the relevant policy makers, educational, medical and hospital managers, according to the findings and also considering the factors affecting death anxiety, educational and interventional programs including in-service programs, psychological training, using the method put multimedia and preparation of educational booklets on the agenda so that in this way it can provide a way for nurses to overcome the existing conditions. One of the limitations of this study is the fact that the questionnaires are self-reported, which can have an effect on the accuracy of the participants' answers, and it is suggested that other data collection methods, such as interviews, be used in future studies.
