اتنوگرافی نهادی؛ رویکردی برای نقد روابط حاکم در موسسات سلامت |

زمینه و هدف: اتنوگرافی نهادی به عنوان یکی از رویکردهای پژوهش کیفی، بر فرآیندهای کاری روزمره افراد متمرکز است. در این رویکرد سوال تحقیق از ناهماهنگی بین آنچه محقق مشاهده می‌کند که در زندگی واقعی افراد اتفاق می‌افتد و آنچه دانش معتبر ادعا می‌کند در حال وقوع است یا باید اتفاق بیفتد ناشی می‌شود. ما در این مطالعه قصد داریم تا به معرفی اتنوگرافی نهادی به عنوان رویکردی برای نقد روابط حاکم بر موسسات سلامت بپردازیم. پژوهشگر تلاش می‌کند تا به محققان و متخصصان مراقبت‌های بهداشتی و درمانی نشان دهد که عملکرد سیستم‌های ارائه دهنده خدمات سلامتی را می‌توان به عنوان سازمان‌هایی اجتماعی تجزیه و تحلیل کرد. در این مطالعه با اصطلاحات، مفاهیم و بینش‌های کلیدی اتنوگرافی نهادی از طریق ذکر نمونه‌هایی از مطالعات منتشر شده، آشنا خواهیم شد.
روش بررسی: این مطالعه روش‌شناسی با جستجو در پایگاه‌های اطلاعاتی PubMed، Scopus، Web of Science،Cochrane Library، با کلمات کلیدی institutional ethnography ، healthcare system ، delivery of health care و در پایگاه اطلاعاتی فارسی زبان مرکز اطلاعات علمی جهاد دانشگاهی با کلیدواژههای "اتنوگرافی نهادی، سیستم مراقبت‌های سلامتی، ارائه مراقبت‌های سلامتی" و همچنین در موتور جستجو گوگل اسکالر، بدون محدودیت زمانی تا بهمن 1402 انجام شد.
یافته‌ها: مراحل انجام اتنوگرافی نهادی را می‌توان طی چهار مرحله تنظیم نمود: "شناسایی گسست و مشکل، جمع‌آوری داده‌ها، تجزیه و تحلیل داده‌ها و توصیه‌ها و نتیجه‌گیری‌های مفید برای تغییر عملکرد". در این رویکرد، اتنوگرافر نهادی به کمک طیف وسیعی از تکنیک‌های جمع‌آوری داده‌ها شامل؛ مصاحبه، مشاهده، گروه‌های متمرکز، جمع آوری مستندات و بازتاب تجربه‌ی خود برای ایجاد توصیفی از آنچه مردم در زندگی روزمره خود انجام می‌دهند، استفاده می‌کند.
نتیجه‌گیری: اتنوگرافی نهادی برای شناسایی و درک اینکه چگونه فرآیندها و سیاست‌های سطح بالاتر بر کار بالینی روزانه تأثیر می‌گذارد، استفاده می‌شود. دست یافتن به یک درک مناسب از چگونگی کارکرد متون و افراد در کنار هم در یک سازمان (چیزی که در حقیقت جریان دارد)، امکان تدوین و اجرای راهکارهای عملی برای حل مشکلات در عرصه را فراهم می‌کند.

Institutional Ethnography; an Approach to Critique the Ruling Relationships in Healthcare Institutions |

Background and Objective: Institutional ethnography, as one of the qualitative research approaches, focuses on the everyday work processes of individuals. This method arises from the research question concerning the discrepancy between what the researcher observes happening in the real lives of individuals and what established knowledge claims should be happening. In this study, we aim to introduce institutional ethnography as a methodological approach for critiquing the prevailing relationships within healthcare institutions. The researcher seeks to demonstrate to healthcare researchers and professionals that the functioning of healthcare service delivery systems can be analyzed as social organizations. This study will familiarize readers with the key terms, concepts, and insights of institutional ethnography through examples from published studies.
Methods: This methodological study involved a search of databases including PubMed, Scopus, Web of Science, Cochrane Library, the Persian Scientific Information Center (SID), and Google Scholar using the keywords "institutional ethnography (IE), healthcare system and delivery of health care" .The search was conducted without any time limitations until January 2024.
Findings: The process of conducting institutional ethnography can be organized into four stages: "identifying the gap and problem, data collection, data analysis, and useful recommendations and conclusions for change." In this approach, the institutional ethnographer utilizes a wide range of data collection techniques, including interviews, observations, focus groups, document collection, and reflexivity to create a description of what people do in their daily lives.
Conclusion: Institutional ethnography is used to identify and understand how higher-level processes and policies impact daily clinical work. Achieving a proper understanding of how texts and individuals function together within an organization (what actually happens) enables the formulation and implementation of practical strategies for addressing issues in the field.
