Peripherally Inserted Central Catheters (PICC) Related to Central Line Associated Blood Stream Infections (CLABSI) and its related factors in the Neonatal Intensive Care Unit |

Introduction: Preventing central line-associated bloodstream infections related to central catheters is a top priority in neonatal intensive care units, as it can reduce mortality, morbidity, and the length of hospitalization. Objective: The current study is conducted to identify factors related to Central Line Associated Blood bloodstream infections (CLABSI) associated with peripherally inserted central catheters (PICC) in hospitalized infants in NICU. Methods and Materials: This study was conducted using a retrospective analytical cross-sectional. The medical records of all infants with PICC were reviewed over 5 years(2018-2023), with the census method, and the data of 321 cases that met the inclusion criteria were extracted. The data collection tool was a neonatal demographic and clinical researcher-made checklist. Mann-Whitney U Test and Logistic regression model were used for data analysis. Results: This study was conducted on infants with PICC, of whom 168 (52.3%) were male and 153 (47.6%) were female. The mean of birth weight was (1219.23±447.68) grams, the mean of weight on PICC insertion day was1215.72±454.81.The mean catheter dwell time was 15.97±10.45 days. The total days of hospitalization of the infants were 40.9+24.7. The results of this study showed that the rate of blood infection caused by PICC is 7.60 cases per 1000 catheter days. The results showed that "total duration of hospitalization" (P=0 .007), " antibiotics therapy in catheter dwell time " (P=0.005), "postnatal age in enteral full-fed (days)" (P=0.002), " mechanical ventilation during catheter dwell time " (P=0.007), " Cause of catheter removal" (P=0.001), "Previous PICC catheter" (P=0.003), "Duration of antibiotic therapy (P=0.005) were statistically significant in infants who had positive blood cultures and those who had negative blood cultures, and the rest of the variables were not different in infants with positive and negative blood cultures. As a result, the chance ratio of positive blood culture increases by 0.06% for every day the infant’s age increases when full-fed enteral feeding is reached. Conclusion: The findings of the present study show that catheter-related bloodstream infections can be influenced by several factors that were investigated in the present study. In addition, the time to full enteral feeding in infants can be predicted as a predictor of increased chances of Blood culture indicated positive. |
