بررسی تاثیر مدت زمان تحریکات حرکتی دهان برای بهبود عملکرد تغذیه ای درکودکان فلج مغزی اسپاستیک |

زمینه و هدف: مشکلات تغذیهای در کودکان فلج مغزی شایع میباشد. مداخلات زود هنگام دهانی حرکتی میتواند عملکرد تغذیهای را در این کودکان بهبود بخشد. هدف از این مطالعه، بررسی تاثیر مدت زمان تحریکات دهانی حرکتی بر عملکرد تغذیهای در کودکان فلج مغزی اسپاستیک میباشد. روش بررسی: دوازده کودک فلج مغزی اسپاستیک در محدوده سنی 2تا 7 سال در دو گروه A , B قرار گرفتند. کودکان گروه A به مدت 12 جلسه(سه روز درهفته) و کودکان گروه B به مدت 24 جلسه(سه روز در هفته) تحریکات دهانی حرکتی را دریافت کردند.مهارت های تغذیه ای با استفاده از مقیاس حرکتی دهان ارزیابی شدند.داده های بدست آمده با استفاده از روش های آماری فریدمن و من ویتنی مورد تجزیه و تحلیل قرار گرفتند. یافتهها: نتایج نشان داد که تحریکات دهانی حرکتی بر عملکرد تغذیهای در هر دو گروه موثر بود. بهبود عملکرد های بستن دهان،بستن لب در هنگام تغذیه با قاشق و مکیدن نی درکودکان گروه B در مقایسه با گروه A تفاوت معنیداری داشتند(05/0 P< ). در عملکردهای جویدن،کنترل مایعات در حین بلع و کنترل مادهی غذایی بین دو گروه تفاوت معنیداری مشاهده نشد(05/0 P> ) .همچنین در نمره کل عملکرد تغذیهای بین دو گروه تفاوت معنیداری مشاهده نشد (05/0 P> )). نتیجهگیری: تحریکات حرکتی دهان بر عملکرد تغذیهای کودکان فلج مغزی اسپاستک موثر میباشد ولی با افزایش تعداد جلسات درمانی (افزیش مدت زمان تحریک) فقط رفتارهای خاصی از تغذیه بهبود مییابند. کلید واژهها: فلج مغزی اسپاستیک، مهارتهای تغذیه ، تحریکات حرکتی دهان. |

Investigation optimal intervention intensity with oral-motor stimulation for feeding function in cerebral palsy |

Background and Aim: Oral feeding difficulties are common in children with cerebral palsy. Early oral motor interventions may improve oral feeding skills in these children. The purpose of this study is to evaluate the effect of intervention duration of oral motor stimulation on feeding skills in spastic cerebral palsy. Materials and Methods: Twelve children with spastic cerebral palsy 2-7 years aged were participated into two groups A and B. Children in the group A received oral-motor stimulation for 12 session(3 days per week) and group B received stimulation program for 24 session(3 days per week).Feeding skills was assessed using Oral Motor Assessment Scale (OMAS). Data were analyzed using Friedman and Mann-Whitney tests. Results : The results of the study revealed a significant improvement in feeding skills for both groups. The B group had significantly advanced in mouth closure, lip closure on utensil and sucking straw than children in group A (P< 0.05). The difference between two group in control of food during deglutition, mastication and control of liquid during deglutition was not significant (P>0.05).There was difference in the total score of feeding skills was not significant between the two groups (P>0.05). Conclusion: Oral-motor intervention might be improved feeding performance in children with cerebral palsy but continuation (increased duration) of intervention improved specific components of feeding behaviors.
