بررسی و ارزیابی محلولهای تزریقی مورد استفاده در تغذیه وریدی طرح ژنریک از نقطه نظر فرمولاسیون و ارزش غذایی درده بیمارستان عمده ایران |

خلاصه: با هدف ارزیابی فرمولهای تغذیه وریدی طرح ژنریک ایران از نقطه نظر فرمولاسیون و ارزش غذائی آنها، این مطالعه بر روی در 31 بیمار بستری در بیمارستانهای امام حسین، خاتم الانبیاء، نیروی هوایی ، امیراعلم، فیروزگر، امیر المؤمنین، دکتر شریعتی، امام خمینی و دکتر رهنمون تهران و بیمارستان سعدی شیراز انجام گرفته است. مقادیر انرژی موردنیاز آنان با روش تئوریک (CEE) و با استفاده از معادله Harris - Benedict و در نظر گرفتن فاکتوراسترس Cerra محاسبه شدند. این نتایج با مقادیر تجربی انرژی مورد نیاز آنان (MEE)از طریق کالریمتری غیر مستقیم مقایسه و نتایج زیر بدست آمد: 1- بدلیل تفاوت در مصرف انرژی زنان و مردان لازم است در فرمول در محاسبه مصرف انرژی دوجنس بیماران بکارگرفته شود، چنانکه در محاسبه مصرف انرژی بازال بافرمول (BEE)Harris - Bendict رعایت می شود. 2- نتایج حاصل از بکارگیری دو روش اندازه گیری مصرف انرژی در بیماران بصورت تئوری و تجربی با یکدیگر قابل مقایسه بوده و می توان دو روش متفاوت را در محاسبه مصرف انرژی بیماران بکار برد. 3- اگرچه فاکتوراسترس Cerra که در برآورد تئوریک مصرف انرژی بیماران بکار می رود، دارای اختلاف معنی داری در مقایسه با فاکتوراسترس واقعی BEE / MEEمی‌باشد ولی از اعتبار نسبی در برآورد مصرف انرژی برخوردار است. 4- به نظر می‌رسد که سرمهای مورداستفاده در تغذیه وریدی بیمارستانهای ایران توانایی نسبی تامین احتیاجات غذایی بیماران را دارا هستند. ولی پیشنهاد می شود که سرمهای قندی تا غلظت 70درصدو سرمهای اسید آمینه مثل Synthaminو Aminoplex برای برطرف کردن نیازهای بیماران در طرح ژنریک ایران در نظر گرفته شده و جهت تهیه آن اقدام گردد. 5- لزوم بکارگیری تیم های تغذیه وریدی متشکل از پزشک، داروساز، متخصص تغذیه و پرستار و تاسیس بخشهای تغذیه و ریدی (Hyperalimentation) در جهت ارتقاء کیفی ارائه سرویس به بیماران توصیه می‌گردد. |

Study and Evaluation of Formulation and Nutritional values of parenteral Nutrition Formulae of Iranian Generic Project in Ten Major General Hospitals |

Abstract: Parenteral Nutrition Formulae in Iranian Generic Project were studied in 31 patients hospitalized in ten major general hospitals in Tehran and Shiraz. Estimated theoretical energy (Calculated Energy Value or CEE) using Harris- Benedict equations and applying the Cerra stress factor was compared with the amount of energy estimated via indirect calorimetric technique (Measured Energy Expenditure or MEE) and following results were drawn: 1 - In view of variation in energy expenditure between males and females, it is necessary to apply two different formulae in calculating the amount of energy expenditure, as it is introduced by Harris- Benedict equations for determination of basal energy expenditure (BEE). 2- Results derived from application of two methods of measurements in determination of energy expenditure (theoretical and experimental methods) were comparable. Thus, it is possible to apply two different methods in measurement of energy expenditure of patients. 3- Although, Cerra stress factor which is applied in theoretical estimate of energy expenditure of patients, is significantly different from true factor (MEE/ BEE), nevertheless.it has a relative validity for estimation of energy expenditure. 4- Formulae used in Iranian Hospitals under present investigation for parenteral nutrition seem to be relatively sufficient for the purpose of provision of patients nutritional needs. However it is recommended that a variety of dextrose formulae in the range of 5-70 percent and other amino acid formulae similar to synthamin and Aminoplex to be supplied in Iranian Generic project. 5- It is strongly recommended that nutrition support teams, comprising of a physician (preferebly a surgeon), a pharmacist. a nurse and a nutritional care specialist, to be established in major Iranian Hospitals in order to assist the management of parenteral nutrition |
