فراوانی اختلالات عملکرد حرکتی دهانی حین تغذیه و برخی از عوامل موثر بر آن در کودکان فلج مغزی |

زمینه و هدف: مشکلات تغذیه ای می‌توانند مشارکت کودکان فلج مغزی در فعالیت‌های روزمره زندگی به‌ویژه غذا‌خوردن را محدود کنند. بنابراین این مطالعه با هدف تعیین فراوانی انواع بازتاب‌های اولیه و اختلال عملکرد حرکتی دهانی کودکان فلج مغزی انجام گردید. مواد و روش‌ها: مطالعه حاضر از نوع توصیفی-تحلیلی به روش مقطعی بود. این مطالعه بر روی کودکان فلج مغزی 2 تا 15 ساله که به‌صورت در دسترس و از نوع آسان از کلینیک‌های توان‌بخشی شهر اراک در سال 1396 انتخاب شدند انجام گرفت. وجود بازتاب‌های غیر طبیعی اولیه شامل بازتاب‌های Bite، Rooting وTongue Thrust و عملکرد حرکتی دهانی کودک با مقیاس ارزیابی حرکتی دهانی (Oral motor assessment scale-OMAS) ارزیابی گردید. جهت تجزیه تحلیل داده‌ها از آزمون خی دو استفاده شد. ملاحظات اخلاقی: این مطالعه با کد اخلاق IR.ARAKMU.REC.1395.82 به تصویب کمیته اخلاق پژوهشی دانشگاه علوم پزشکی اراک رسیده است. یافته‌ها: نتایج نشان داد بیشترین تعداد افراد دارای بازتاب‌های اولیه دهانی غیرطبیعی (Bite، Rooting و Tongue Thrust) در سطح IV و V عملکرد حرکتی درشت و در سطح III توانایی دستی و نوع کوادروپلژی اسپاستیک قرار دارند. بیشترین فراوانی فقدان عملکرد در مهارت حرکتی دهانی در سطح عملکرد حرکتی درشت V و عملکرد کامل در عملکرد تغذیه دهانی در سطح عملکرد حرکتی درشت I و II مشاهده گردید. بین مهارت حرکتی دهانی با سطح عملکرد حرکتی درشت اختلاف معناداری مشاهده شد (04/0= p). نتیجه‌گیری: مطالعه نشان داد که تفاوت معناداری بین عملکرد حرکتی دهانی و سطوح عملکرد حرکتی درشت وجود دارد. بنابراین پیشنهاد می‌گردد که در ارزیابی عملکرد حرکتی دهانی کودکان دارای فلج مغزی، توجه خاصی به سطح عملکرد حرکتی درشت این کودکان شود. |

Frequency of Oral Motor Dysfunction during Feeding and Some of Its Effective Factors in Children with Cerebral Palsy |

Background and Aim: Feeding problems can limit the participation of children with cerebral palsy (CP) in the daily activities of life, especially feeding and eating. The aim of this study was to determine frequency of primary oral reflexes and oral motor dysfunction in children with CP. Materials and Methods: This study was a descriptive-analytic and cross-sectional study that has been done on children with CP aged 2-15 years old. The participants were chosen by a convenience sampling from the rehabilitation clinics in the city of Arak in 2017. The presence of primary abnormal oral reflexes including Bite, Rooting, and Tongue Thrust was evaluated; and Oral motor assessment scale was used to evaluate oral motor function.Data were analyzed using statistical tests including chi-squared test. Ethical Considerations: This study with research ethics code IR.ARAKMU.REC.1395.82 has been approved by research ethics committee at Arak university of medical sciences, Iran. Findings: Based on the present results, the most subjects with primitive oral reflexes (bite, rooting, tongue thrust) were found at levels of IV and V Gross Motor Function Classification System Expanded &Revised(GMFCS E&R) and at level of III Manual Abilities Classification System and type of quadriplegia spastic. The highest frequency of passive in oral motor was observed in level of V and functional in oral motor in levels of I and II GMFCS E&R. Furthermore, there was significant difference between gross motor function and oral motor function. Conclusion: The study results demonstrated a significant difference between GMFCS E&R levels and oral motor function. Therefore, it was suggested that particular attention should be paid to oral motor function in terms of evaluations of gross motor function. |
