Background: Mortality from cardiovascular causes, including pulmonary embolism, is the second most common cause of mortality in psychiatric patients. Sign and symptoms which are specific for PE include tachycardia, dyspnea. It rarely manifests as acute psychosis and other unspecific sign and symptoms. Many common treatment modalities for psychiatric disorders increase the risk of coagulopathies and there is some situation such as catatonia which are associated with an increased incidence of VTE. PE is preventable cause of death for psychiatric patients. So we present this case to emphasize on the importance of VTE complications in psychiatric patient. Case summary: A 56-years-old woman, was admitted because of aggression, restlessness and disturbance since one week before admission. She was hospitalizing more than 6 times “with diagnosis of bipolar spectrum”. On the basis of available information, a diagnosis of “Unspecified Bipolar and Related Disorder” with akathisia (possibly doxepin and SSRI induced) was considered. doxepin, citalopram and olanzapine gradually were tapered. also tablet sodium valproate 200mg/QID and tablet propranolol 20 mg/Bid was added to her drugs. After two weeks, with no progress in relieving symptoms, tachycardia, was detected. So consultation of an internal medicine specialist and more evaluation was requested. With high level of D-dimer, and low saturation, we considered thromboembolism, the patient was sent to a general hospital, then diagnosis of VTE was confirmed. Conclusion: one of the most important cause of death in psychiatric ward is VTE. It is difficult to diagnose the VTE in psychiatric patient, so it could increase the risk of mortality. Therefore, it is necessary for psychiatrists to perform the essential assessments for patients with clinical suspicion. We publish this case in order to raise the awareness of thromboembolic complications in psychiatric patient, specially hospitalized patients. |
