عنوان انگلیسی The Effect of Cervical Lordosis Restoration on Global Spine Alignment Following ACDF Based on EOS Imaging

Objective: Reciprocal alterations in cervical spine alignment and global spine alignment is well acknowledged. However, there is a small level of evidence regarding the alterations in global spine alignment following the restoration of cervical lordosis following anterior cervical discectomy and fusion (ACDF). In this study, we evaluate the impact of ACDF on the global spine alignment.

Patients and methods: Thirty-two patients with a mean age of 46 ± 10 who underwent ACDF for cervical disc herniation were included in this retrospective study. Cervical alignment and parameters of local and global spine alignments and pelvic parameters were evaluated on the preoperative and three month postoperative EOS® images.
Results: All the spinal indices were altered after the operation, and this difference was statistically significant in all cases except for thoracic kyphosis. Three months after the ACDF, total cervical lordosis correction was higher in patients with a single fusion level (P<0.001), while the local cervical lordosis correction was significantly higher in patients with more than one fusion levels (P<0.001). Pelvic tilt change was significantly more in patients with more than one fusion level (P=0.043).
Conclusion: In patients undergoing ACDF despite the number of fusion, restoration of cervical global and local lordosis change the alignment of the global spine.
