بررسی ارتباط دیابت بارداری با آپگار نوزادان |

مقدمه: دیابت دوران بارداری می‌تواند پیامدهای مضری بر سلامت نوزادان داشته باشد. ازآنجا که اولین شاخص بررسی سلامت نوزادان سیستم آپگار می‌باشد، لذا این مطالعه با هدف بررسی ارتباط دیابت بارداری با آپگار نوزادان طراحی شد. روش کار: مطالعه حاضر از نوع مطالعات مورد - شاهدی بوده است که50 نوزاد از مادران مبتلا به دیابت بارداری و 100 نفر از نوزادان مادران سالم که طی سالهای 84 تا 89 در بیمارستان حضرت ولی عصر شهرستان فسا متولد شده بودند، بر اساس معیارهای مطالعه و به صورت تصادفی انتخاب شدند. گروههای فوق از نظر سن حاملگی هماهنگ بوده و از نظر مشخصات جنینی و آپگار دقیقه یک و پنج تولد مورد مقایسه قرار گرفتند. آپگار نوزادان مادران سالم و مبتلا به دیابت بارداری با استفاده از آزمون تی مقایسه شد. با استفاده از رگرسیون چند متغیره رابطه آپگار نوزاد و دیابت بارداری پس از حذف اثر متغیرهای مخدوش گر بررسی شد. نتایج: میانگین سن مادران مبتلا به دیابت بارداری 0/5±5/31 سال و گروه کنترل برابر با 3/6±6/27 سال بود. همچنین میانگین سن حاملگی گروه مبتلا به دیابت بارداری 6/1±9/37 هفته و گروه کنترل برابر با 7/1±3/38 هفته بوده است. آپگار دقیقه اول نوزادان مادران با دیابت بارداری 6/0±6/8 و در مادران سالم 7/0±7/8 بود (157/0=P). اما آپگار دقیقه پنج در نوزادان مادران دیابتی 6/0±5/9 به طرز معنی‌داری کمتر از آپگار نوزادان مادران سالم 4/0±8/9 بود (002/0=P). آنالیز رگرسیونی نشان داد که تفاوت آپگار دقیقه 5 بین مادران گروه دیابت بارداری و گروه کنترل، تحت تأثیر متغیرهای مخدوشگر نبوده و تنها به دلیل ابتلای مادر به دیابت بارداری می‌باشد. نتیجه‌گیری: از آنجا که آپگار دقیقه پنجم در نوزادان مادران دیابتی به طرز معنی‌داری کمتر از آپگار نوزادان مادران سالم است و افت شاخص آپگار به برخی از ناهنجاری‌ها و مشکلات در نوزادان منجر خواهد شد، لذا توجه هرچه بیشتر به نوزادانی که از مادران مبتلا به دیابت بارداری متولد می‌شوند در جهت ارتقای سلامت، مفید خواهد بود |

A Survey of the relationship of Gestational Diabetes Mellitus on Neonate’s Apgar score |

Introduction: Gestational Diabetes Mellitus (GDM) may have harmful effects on neonate health. Since Apgar score is the first investigation indicator of neonate health, this study was conducted to investigate the relation between GDM and neonate Apgar score. Methods: This study is a case-control one, in which 50 neonates from mothers suffering from GDM and 100 neonates from healthy ones born in 2005-2010 in Vali-e-Asr hospital, Fasa, Iran, were selected randomly based on their medical file numbers. The cases were homogeneous regarding gestational age and were compared regarding their fetal features and apgar score in the first and fifth minutes after the birth. Then, Apgar score of the neonates from healthy mothers and those suffering from GDM were compared by T-test. Then using multivariate regresion, the relation between GDM and the neonate Apgar score were investigated after omitting confounder factors effect. Results: The average age of the mothers suffering from GDM and control group were 31.5±5.0 and 27.6±6.3 years old, respectively. Also, the average gestational age of the former and the later were 37.9±1.6 and 38.3±1.7 weeks, respectively. Apgar score of the first minute in the neonates of the former and the later were 8.6±0.6 and 8.7±0.7, respectively (P=0.157). But, Apgar score of the fifth minute in the former 9.5±0.6 was significantly lower than the later 9.8±0.4 (P=0.002). Regression analysis showed that the difference between Apgar score of the fifth minute in both groups wasn’t affected by confounder variables and this was just due to GDM. Conclusion: Since Apgar score of the fifth minute in the neonates from the mothers suffering from GDM was significantly lower than those from healthy ones and due to the fact that apgar score indicator decrease leads to some disorders and problems in the neonates from the mothers suffering from GDM, taking the neonates into consideration as much as possible would be helpful in improving health level |
